Access Your Medical Records

Sign in to your patient portal account to access your medical records.

Using your patient portal accounts you can receive a copy of your reports, message your provider, access MyChart and Athena Health, see your last visit note and review your latest testing and lab results.

To receive a copy of your medical record, including immunization records, please complete the fillable release form below and send it back to our office. 

You may email, fax or send the completed form using the information listed below. Please ensure that you provide your full name, date of birth, the last 4 digits oinlucdif your social security number (SSN), indicate the specific records needed, and how you want the records to be delivered to you. If you would like to receive your medical records electronically, you’ll also need to provide your current email address.

Immediate Access to your Electronic Medical Records

In 2016 Congress passed the bipartisan 21st Century Cures Act to promote the electronic access, exchange and use of health information. As a result of this federal mandate, Chesapeake Regional Healthcare now releases clinical test results and clinical notes placed in the patient’s electronic medical record as soon as results are published, with the following exceptions (which must be documented for auditing purposes):

  • Psychotherapy notes deemed harmful to the patient
  • Notes related to a civil and/or criminal proceeding
  • A state law prohibiting the release of certain test results (based on Commonwealth of Virginia law, HIV results cannot be auto released  

What is being shared?

Patients with access to their electronic medical records via MyChart will receive their clinical test results as soon as they are published. They will see clinical notes, in some cases, prior to conversations with their providers who typically explain the complexities of a diagnosis. 

Patients now have access to the following:

  • Consultation Notes
  • Discharge Summary Notes
  • History and Physical Notes
  • Procedure Notes
  • Progress Notes
  • Imaging Narrative
  • Laboratory Report Narrative
  • Pathology Report Narrative

The reasoning behind the 21st Century Cures Act

Market research has confirmed that consumers are interested in receiving test results as soon as they are available. This new rule is intended to enhance transparency, patient/provider trust and confidence in care through a greater partnership of care treatment and delivery. 

Patients want immediate notes/test results for the following reasons:

  • Helps patients understand the “why” behind their treatments, thus more willing to be adherent to the treatment plan
  • Deepens trust between patient and provider as it furthers the partnership of care between the two individuals
  • Enhances the mutual respect between patient and provider
  • Allows patients to share more information with family, spouses et al, which could help caregivers understand the treatment plan which they are supporting
  • Allows patients to share their data with other providers who might not be on EPIC
  • Removes the stress and relieves the anxiety from patients awaiting a test result
  • Allows patients to have a greater sense of ownership of their results and notes
