A Community Health Needs Assessment (CHNA) examines the overall health of a population while identifying key assets and challenges that influence well-being. The process engages community members and local public health partners to collect and analyze data from a wide range of sources. This approach prioritizes health services and the strategies to address identified gaps. 

In short, a CHNA provides a roadmap for improving community health by combining data, community voices, and cross-sector collaboration to drive meaningful, equitable change.

This CHNA focuses on the City of Chesapeake, Virginia, and eleven counties in northeastern North Carolina. It is a collaborative effort between Chesapeake Regional Healthcare, the Chesapeake Health Department, and Healthy Chesapeake and is updated every three years.
