The Transitional Care Clinic Is Selected As A Health Quality Innovator Of The Year For Virginia

Award recognizes The Transitional Care Clinic’s dedication to improving health in the Chesapeake community


Chesapeake, Va.— Health Quality Innovators (HQI), an independent, non-profit consulting organization, awarded Chesapeake Regional Healthcare’s Transitional Care Clinic (TCC) as a Health Quality Innovator of the Year for Virginia during a luncheon held in Richmond on Oct. 24. It was designated as the winner of the Collaboration category for its dedication to bringing others together to solve health challenges.
 
The TCC at CRH is a high-touch integrated practice model that provides access to care for high-risk, high-needs patients throughout the city of Chesapeake, Va. This unique, integrated health care clinic works in collaboration with the Chesapeake Care Clinic, the Liberty Street Clinic, ComfortCare Home Health and Hospice, the Virginia Department of Health, Healthy Chesapeake and the Graduate Counseling Program at Old Dominion University to break down silos and eliminate barriers to care. 

“We’re extremely proud of the Transitional Care Clinic for its recognition as a Health Quality Innovator of the Year,” said Reese Jackson, President & CEO of Chesapeake Regional Healthcare. “This represents a truly collaborative effort among an incredible team of people working to improve the health of this community.” 

At TCC, patients and their families receive in-depth education, an assigned social worker to assist with resources and behavioral health counselors to help work through external stressors. All of these health care services they would have otherwise received through an emergency department visit. 

The TCC model has reduced hospital readmissions (8 percent 30-day hospital readmission rate) and emergency department visits (43 percent reduction in inpatient hospitalizations) in the Chesapeake community. 

The Health Quality Innovator Awards are HQI’s marquis program for gathering and sharing evidence-based, practical methods for enhancing quality and outcomes across physician practices, hospitals, long-term care facilities and other settings.
