Access MyChart

To access patient portal, you will be provided a MyChart activation code at the back of your After Visit Summary at the time of discharge.

With MyChart, you can:                                                         

  • View your health summary
  • Access your test results & notes
  • View your recent hospital & outpatient visits
  • Complete online questionnaires
  • See scheduled appointments and reminders
  • Schedule appointments and video visits [where applicable]
