Diabetes Self-Management EducationAmerican Diabetes Association Certfication

The Diabetes and Nutrition Program at Chesapeake Regional Medical Center's Lifestyle Center has received national recognition by the American Diabetes Association for being a quality   diabetes education program since 1991. To help you learn how to manage your diabetes, we offer individual counseling with certified diabetes nurse educators and dietitians, group diabetes education classes called "Diabetes University" and carbohydrate counting classes. To enroll, your provider must fax a referral, or you may contact us yourself. Services provided to adults aged 18 years or older. Participation will require a referral from your provider and this program is usually covered by insurance. We offer a diabetes support group for women called “Diabetes Sisters. Nutrition Consults

We also offer individual nutrition consults or Medical Nutrition Therapy for a variety of medical conditions:

  • Diabetes
  • PreDiabetes
  • Carbohydrate counting
  • Weight management
  • Reactive Hypoglycemia
  • Hypertension
  • Elevated blood cholesterol or lipids
  • Cardiovascular disease
  • Chronic Kidney disease
  • Celiac Disease
  • Other Medical Conditions

Diabetes University

If you're looking to continue your diabetes education, enroll in Diabetes University. A multidisciplinary team of health care professionals teaches the three-part program, open to all diabetes patients and their families. Topics include nutrition, diabetes management, foot care, complications, medications, community resources and more. Most insurance companies will cover the cost. Call 757-312-6137 to register.
